Use this Photo Release Form if:
You are a photographer who would like the rights to photographs of models or other individuals in order to publish content
You are a model and you wish to transfer the rights of your photographs to someone else.
Note that for models under the age of 18, a legal guardian must countersign the form on the child’s behalf. The forms are encouraged to be in writing.
